CALLING ALL CASE MANAGEMENT NURSES!!!
(Multiple Openings) RN Care/Case Manager
Permanent Position!
Boston, MA
RN Case Manager needed to train family members as caregivers for their family member needing healthcare at home.
Experience
At least 2 years of Case Management experience, preferably in a home health setting.
  • 1x per month visit to the patient s home. Evaluate and record patient info.
  • Regular virtual coaching meetings with the family to train, answer questions, and assist in home healthcare.
  • Shift: Monday Friday, flexible schedule working with the family s needs (no nights, no weekends).
  • Full-time, 40 hours/week.
  • 50-55 Patient cases at a time.
  • Competitive Salary for a state-funded company
Requirements
  • 2+ Years recent experience in direct care of elders or adults with disabilities.
  • Active RN Licensure in state of employment
  • Ability to travel (Driver s License with auto insurance required)
  • Tech proficiency, laptop use and web-based applications
